Become a Provider
Becoming a participating provider in the Chicago Health System network begins with completion of a pre-application. A pre-application can be obtained by contacting the Chicago Health System Credentialing Department if you are practicing under an already contracted organization. If you are a new organization to Chicago Health System or an independent practitioner contact the Chicago Health System Contracting department.

Preliminary Requirements to Join our Network
Board Certification in Practicing Specialty through the American Medical Association, American Osteopathic Associates, American Board of Medical Specialties, American Board of Pain Medicine, American Board of Physician Specialties, American Board of Podiatric Medicine, American Board of Podiatric Surgery or American Board of Psychiatry and Neurology.
Admitting Privileges at one of our 4 participating facilities: MacNeal Hospital, Westlake Hospital, Weiss Memorial Hospital, West Suburban Medical Center.
Office Hours for Primary Care Providers are required to include a minimum of 8 hours per month outside the hours of 9:00am – 6:00pm Monday – Friday.
Board Certification through the American college of Nurse Midwives, American Nurses Credentialing Center, American Academy of Nurse Practitioners, American Midwifery Certification Board, Pediatric Nursing Certification Board, National Certification Corporation (Women’s Health Nurse Practitioners and Neonatal Nurse Practitioners only), or Accreditation Board for Specialty Nursing Certification.

Board Certification through the National Commission on Certification of Physician Assistants.
